Appointment Process:

• You will receive an immediate automated email response from Lifegate.
• Our Administrator will contact you via text to set up the appointment.
• You will be offered an appointment date and time.
• You are responsible to confirm the appointment with our admin via text.
• A confirmation email will be sent to you with important documents for you to read prior to your appointment.
• Location and directions to our office are included in the email and documents.
• Due to our waiting list, it may be several weeks between your confirmed appointment and your initial request.
